Member health services who register and join up to the Hero Rewards program are rewarded with large incentive payments for registering, identifying and treating community members who are living with chronic diseases (e.g. diabetes, mellitus, asthma, cancer, cardiovascular illness, musculoskeletal conditions, strokes and other chronic conditions).

The Indigenous Chronic Disease Package (ICDP) will be administered through an incentive payment system. Medicare will pay a one-off sign-on amount, followed by monetary rewards to services on a per-client basis for the provision of best-practice care and chronic disease management.
